IRMAA — the Income-Related Monthly Adjustment Amount — is a surcharge that higher-income Medicare beneficiaries pay on top of the standard Part B and Part D premiums. For 2026, the standard Part B premium is $202.90 per month, and IRMAA can push it up to $689.90, plus a Part D surcharge of up to $91.00.

The surcharge is based on your modified adjusted gross income (MAGI) from two years earlier — so your 2026 premium is set by your 2024 tax return (AGI plus tax-exempt interest). Crossing a bracket by even $1 bumps you into the next tier for the whole year, which makes IRMAA a classic 'cliff.'

How IRMAA works in 2026

Base Part B premium (2026) = $202.90/month
IRMAA tier                 = based on your 2024 MAGI and filing status
Part B total               = base + Part B IRMAA surcharge
Part D                     = your plan premium + Part D IRMAA surcharge

Your 2026 IRMAA uses your 2024 MAGI = AGI + tax-exempt interest. The Social Security Administration mails an IRMAA determination in late 2025 based on that return.

For married filing jointly, the income thresholds are double ($218,000 up to $750,000) with the same premium amounts. Married filing separately has its own steep schedule: over $109,000 jumps to $649.20 Part B, and over $391,000 to $689.90.

The IRMAA 'cliff'

IRMAA is not phased in gradually. Go $1 over a threshold and you pay the higher tier's full surcharge for the entire year. That is why retirees watch the edges of each bracket when timing Roth conversions, capital gains, and IRA withdrawals.

Two-person households

Each spouse on Medicare pays IRMAA individually. A couple both in Tier 5 pays the surcharge twice, so the household cost is roughly double the per-person figures above.

Appealing IRMAA

If a life-changing event (retirement, divorce, death of a spouse, loss of income) dropped your income after 2024, you can ask SSA to use more recent income with Form SSA-44.
